Community Care Services Manager in Brisbane Northern Suburbs

The Group:

Responding to the rapidly growing home care and support services market and partnering with a diverse group of stakeholders, which include Government, Individuals, Retirement Village Operators, Health Care Professionals and Hospitals to deliver quality of life and in-home care services, the need to appoint this integral role to support the community is actively on the Group’s hiring agenda.

The Role Brief:

With a flat management structure, you will report directly to the General Manager and work closely with the Community Manager to coordinate client services and care through case management, staff allocation and rostering, ensuring an appropriate mix of support services are delivered which are responsive to the individuals needs and consistent with the Community Care Common Standards.

You will enjoy consulting with Clients, significant others to design, monitor, review and coordinate care plans following comprehensive care assessments.  You will consult with Registered Nurses on case and care matters, engaging clinical nursing expertise to assess, plan, monitor and review complex clinical care elements along with but limited to;

Working with the Executive Manager suggesting options and opportunities for the development and expansion of the client services business.

Your Success:

Critical to your success and pivotal to this role is your desire to work with the aged, your advanced interpersonal communication skills; accuracy in maintaining and managing comprehensive and confidential client files – consistent with legislative and regulatory requirements, stakeholder engagement experience; the ability to manage, develop and mentor your team and build culture; your Qualifications around Community Services or equivalent discipline coupled with your demonstrated experience and knowledge of Community Care Standards, Residents Rights and Responsibilities and delivering outcomes in a client centred, community-based care service delivery environment.
